No public patient account
Consultation and assessment forms create an enquiry record for clinic follow-up; they do not create a login, profile or public patient account. Therefore, there is no online account dashboard to close.
How to request deletion
Complete the dedicated deletion-request form below. Provide the name and at least one contact detail used in the original submission so the clinic can locate the relevant record and verify the request.
Identity verification
To protect your information, the clinic may ask for proportionate verification using the contact details associated with the request. Do not send identity documents unless specifically requested through an appropriate channel.
What will be deleted
After verification, website enquiry records and associated contact details that are no longer required will be deleted or anonymised. The clinic will also take reasonable steps to address copies held by relevant processors where applicable.
Information that may be retained
Deletion may be limited where information must be retained for legal, regulatory, clinical, safety, fraud-prevention, dispute or record-keeping obligations. Any retained information will remain restricted to those purposes. Clinical records created after a consultation are not the same as a website enquiry and may have mandatory retention periods.
Confirmation and further requests
The clinic will respond within the period required by applicable law and will explain if further information or a lawful retention exception applies. You may also request access or correction using the same process. Review the Privacy Policy for more information.
